AlviPsych LLC – Terms of Use

1. Platform Role & No Medical Care

AlviPsych LLC (“AlviPsych”) is a technology and administrative platform that facilitates access to psychiatric evaluations performed by independent licensed healthcare providers.

AlviPsych does not practice medicine, does not provide clinical care, and does not control or direct the clinical judgment of any provider.

2. No Therapeutic Relationship

Use of the AlviPsych platform does not create a doctor-patient, therapist-patient, or treating relationship between you and AlviPsych.

Any clinical relationship that may arise with a provider is limited solely to a one-time, non-therapeutic evaluation, as described in the Informed Consent.

3. Not for Emergency or Crisis Use

The AlviPsych platform is not intended for emergency or crisis situations.

If you are experiencing suicidal thoughts or a psychiatric emergency, contact 911, a local emergency department, or a crisis hotline immediately.

4. User Responsibilities

You agree to:

• Provide accurate and complete information
• Use the platform only for lawful purposes
• Seek ongoing medical or psychiatric care independently
• Follow your surgeon’s and treating providers’ recommendations

5. No Guarantee of Surgical Clearance

AlviPsych does not guarantee surgical clearance or a favorable evaluation outcome.

All clearance determinations are made by independent providers, and all surgical decisions are made by surgeons and facilities.

6. Fees, Payment & No-Refund Policy

All fees are self-pay unless otherwise stated.

No refunds are issued for missed appointments, late arrivals, or same-day cancellations, regardless of reason.

7. Authorization to Share Information

You authorize the evaluating provider, through the AlviPsych platform, to share evaluation reports and relevant information with your surgeon and surgical facility via secure electronic means.

8. Limitation of Liability

To the fullest extent permitted by law, AlviPsych shall not be liable for clinical decisions, surgical outcomes, or your failure to seek appropriate care.

9. Indemnification

You agree to indemnify and hold harmless AlviPsych LLC and its affiliates from claims arising from your use of the platform.

10. Arbitration & Governing Law

Any dispute shall be resolved by binding arbitration, unless prohibited by law.

These Terms are governed by the laws of the state in which the evaluation is performed.

11. Modifications

AlviPsych may modify these Terms at any time. Continued use constitutes acceptance.

12. Severability

If any provision is unenforceable, the remaining provisions remain in effect.

13. Entire Agreement

These Terms, together with the Informed Consent and Privacy Practices, constitute the entire agreement between you and AlviPsych.
